How to format a phplist newsletter
First of all click to send a new message in phplist.
Then type in the text of the newsletter. Try to avoid pasting in from MS Word - copy from a TXT file if possible. Format the headers in the text using the style 'Heading 2' as shown in the small menu.

Make the contents list at the top using the bullet list option:

Then add Anchors into the text as shown. NOTE: this needs to be done FIRST, before links are added to the contents list.

Then go to the contents list, highlight a menu option, and then click the link icon as shown:

When you click the link icon, a window pops up. Fill it thus:

Next, images...
Rather annoyingly phplist does not have an image upload feature. So instead you need to get images from somewhere else on the web, or perhaps images you have previously added into Wordpress. Right click any image in your browser and you get a small menu like this one popping up. Choose 'Copy Image Location' or the equivalent from the menu:

Then back in phplist click the small icon with the brown mountain on it (not shown below) and a popup window (as shown) will appear. Fill in each of the fields as shown:

Repeat this process for further images.
Then at the very bottom of the screen in phplist save your message, and send a test message before sending out the message:

Lastly check the test message looks right, with the colours from the template applied to your message:
